Please answer the following questions. A negative answer may preclude a performance at your school due to safety concerns. A mandatory site visit will occur several weeks prior to a performance to ensure meeting safety criteria.
Does your school have a staging area for the performance? Such as an auditorium or gymnasium?
Does the staging area have access to emergency exits?
Is the staging area located at least four meters away from the students/spectators?
Is there at least two power supplies adjacent to the staging area?
Can the staging area accommodate two (2x) two-meter folding tables?
Is there access to a water source, such as a bathroom sink, near the staging area?
Is there a loading/unloading area near the staging area?
Is there adequate ventilation in the staging area?
Can the staging area be segregated by teachers or administrators so that no students are allowed to enter the staging area unless under the supervision of the staff?
Can the areas directly to the right and left of the staging area be kept clear of students or spectators?
Can the staging area be accessed at least 90 minutes prior to the performance time?
Can a teacher or administrator assist students in donning safety gear; such as lab coats, safety goggles, or gloves?
The shows should be performed in your auditorium or large assembly area in order to maximize the number of students, as long as safety measures and safety exits are kept clear. Our recommended maximum capacity for one show would be between 150-200 students of adjacent grade levels.
The information submitted with this application form is accurate and complete.
